URGENT: Return to School Health Form

Dear Spartan Families,

As we prepare to return to school this fall, we have additional required health forms and information related to the current pandemic.  Specifically:

1.      COVID-19 Acknowledgement of Risks

2.      Health guidance and daily self-assessment acknowledgment

3.      Changes to medical policies as a result of COVID-19

4.      Immunization information

To simplify the completion of this process, we've created a single online form where you can read and sign each of these forms:

Please read and sign the form BEFORE sending your child to school.  If you would prefer a paper copy, please contact your school office to schedule an appointment to do so.
